THE ATHLETE HOUSE MOBILE APP
The following additional terms apply to the The Athlete House mobile application ("App"). The general practices above apply to the App as well; this section adds App-specific details.
INFORMATION COLLECTED THROUGH THE APP
When you use the App, we may collect the following types of information:
Account Information: Your name, email address, and optional phone number, provided when your provider invites you to use the App.
Care Plan Information: The exercises your physical therapist assigns to you and your progress in completing them.
Messages: The content of messages you exchange with your provider through the App.
Profile Photo: If you choose to upload one.
Device Information: A push-notification token tied to your device, so we can alert you to new messages and assignments. We do not collect your device location through the App.
HOW WE USE APP INFORMATION
We use the information collected through the App to:
• Deliver your personalized care plan and let you communicate with your physical therapist
• Send App notifications about messages, exercises, and appointments
• Maintain, troubleshoot, and improve the App
• Comply with applicable laws and regulations
We do not sell App information, share it for advertising, or use it to track you across other apps or websites.
APP SERVICE PROVIDERS
We use trusted third-party service providers to operate the App — including cloud hosting and authentication, app delivery, push-notification delivery, appointment scheduling, and clinical record systems. These providers process information only on our behalf and under appropriate confidentiality and security obligations.
APP-SPECIFIC CHOICES
Push Notifications: You can disable push notifications at any time in your device's Settings app.
Account Deletion: You can delete your App account from inside the App (Profile → Delete Account). Some clinical records may be retained as required by Kentucky physical-therapy record-retention rules.
Children's Use: The App is not intended for use by individuals under 13 years of age.
